HOME HEALTH CARE- EXTENDED CARE OVERVIEW:

Henry Ford Extended Care private-duty services provide individualized care for clients in the home or facility setting. Services range from pediatric to geriatric and provide long or short term care - from basic personal hygiene and housekeeping assistance provided by a nurse aide to specialized care requiring the skills of a licensed nurse or therapist. Respite care, companionship services, errand services and worksite health programs, including flu immunizations and hepatitis B vaccines, are also offered.

Private duty services are also available for clients in facility settings such as hospitals, nursing homes or rehabilitation centers. Many individuals find this aspect of the service helpful to ensure that full comfort measures and safety needs of the client are met.

Responsibilities

- Under general supervision from the Registered Nurse, utilizes the knowledge and practical experiences of a licensed practical nurse to provide direct patient/family care in the home, facility, or school.
- LPN collects data and collaborates observations to the R.N. and participates in the development of the plan of care.
- Performs a variety of both technical and routine nursing services.
- Responsible for the skillful performance of nursing procedures.

Qualifications

Required Qualifications:

Requires graduation from an accredited school of practical nursing, current licensure as a practical nurse in the state of Michigan,

NAPNES certified

Minimum of 1 year previous LPN experience.

Must have reliable transportation and a telephone, a valid State of MI driver's license and an acceptable driving record.

Must exhibit excellent interpersonal skills , history, good work ethics practice and customer -focus behaviors

Preferred Qualifications:

At least 1 year experience in private duty or home care

Pediatric experience a plus (for school placement)

Current BLS
