Responsibilities
* Utilizes the knowledge and practical experiences of an LPN to provide direct patient/family care in the home, traveling at least 50-75 miles per day.
*
Perform within the scope of practice and standards of practice as legislated by state and federal government agencies which represent the foundation for delivery of safe, competent and cost effective nursing practice, to safeguard the physical, mental, emotional and spiritual health of the patient and his family.
* Under minimal supervision of provider or RN, performs direct patient care services and technical support procedures in a multi-task oriented environment, follows prescribed department protocol.
* Complies with all HFHS and departmental specific safety standards including Universal Precautions, Electrical Safety, Infection Control, Fire Safety, and Emergency Procedures.
Qualifications:
Education/ Experience Required:
* Successful completion of an accredited LPN nursing program
* Licensure by the State of MI as an LPN.
* NAPNES certification or successful completion of pharmacology exam
* At least 2 years clinical medical-surgical or home health care experience as an LPN
* Ability to effectively communicate with patients, families and health care personnel
* Must have reliable transportation and possess a current State of MI driver's license, and a satisfactory driving record.
Certifications/Licensures Required:
* Current LPN license in the State of Michigan.
* Complete BLS-C Course and demonstrates knowledge of basic emergency procedures.
